Clarification from Slack
It seems there’s some confusion with the post from this morning (2/25). I know there is a lot to track ahead of a trip like this and hopefully this helps!
- Agreement Form: This was the first document we posted that needed attention. It houses the Trip Guidelines and lays out nearly everything you need to know. The link is what you need to PRINT, sign and turn in to me (every student in band, choir, orchestra).
- Medical Release Forms:
- Orchestra and Choir (you do not have the option to skip posting a copy of your insurance card)
- Band (this is the beginning of the year member registration) - also, it doesn’t give you the option to NOT post your insurance card. For most of you, it was filled out in July or August. If not marked on the roster, we don’t have your information. Please fill it out.
- Medications and Allergies: This is the one generating the most confusion. If you’re on ANY kind of prescription meds, you need to fill this out. If you’re not, you don’t.
